Initiation of Agreements
Each agreement originates during a personal interview between a prospective tenant/resident, their family (or personal representative) and a HOMEPLACE staff member.  In this interview, the tenant and their family determine the services to be provided.  This agreement is amendable as needed but carries the understanding that it will be automatically renewed each successive calendar month until terminated or amended by the Tenant or The HOMEPLACE.

Circumstances where Assisted Living Agreements may terminate but Dementia care may continue
When the need for personal care increases, HOMEPLACE services adapt as well.  It is most likely that the decision to come to The HOMEPLACE will be the last moving decision that someone needs to make.  However, within the Assisted Living units, there are some circumstances that would require termination of our service agreement with you.

They are as follows:
- Supportive, personal, and nursing care exceeds 28 hours per week.  This limit is set by state statute. The exception would be when additional hours of service are negotiated with an independent caregiver approved by The HOMEPLACE, such as with a Hospice agency.
-A tenant's behavior or condition poses an ongoing threat to the health or safety of themselves or others.
- A tenant is adjudicated incompetent under Wisconsin Statute Chapter 880.*
- A tenant's condition requires the immediate availability of a nurse 24 hours a day.
- A tenant refuses to cooperate during an examination by a physician or psychologist of his/her choosing to determine his/her health or mental status for the purpose of establishing the appropriateness for retaining or terminating The HOMEPLACE agreement.
- A tenant's fees have not been paid, after The HOMEPLACE has given reasonable opportunity to pay any deficiency.
- A tenant refuses to enter into or revise a Negotiated Risk Agreement when there are significant medical or safety reasons for doing so.

*Note, though an Assisted Living Apartment may no longer be appropriate for a tenant, a relocation to one of our Haven Alzheimer's & Dementia Units MAY BE A POSITIVE ALTERNATIVE.

We do everything we can to encourage and promote each tenant's independence. Yet, the bottom line is safety.  If the time comes when someone can no longer participate here in planning for themselves, professional staff at The HOMEPLACE will work closely with that person's family and the person holding Power of Attorney to safeguard that tenant's future.
